Forms

The Sign Language Interpreter & Transliterator State Registration Act has eight exceptions outlined in Section 4 (b). In order to be in compliance with Exceptions (b)(4) and (5), completion of the appropriate form(s) in its entirety is required. Forms may be obtained by clicking on the link or by contacting ODHH.

Section 4(b)(4)

This exception allows interpreters / transliterators certified by the Registry of Interpreters for the Deaf or the National Association of the Deaf who do not reside in the commonwealth to provide interpreting or transliterating services in Pennsylvania for a period not exceeding 14 days per calendar year. To comply with this exception, complete the following form: Out-of State Certified Interpreters / Transliterators Working in Pennsylvania.

Section 4 (b)(5)

This exception allows a person who is deaf, deafblind or hard of hearing to request the services of a non-registered interpreter. Once the request is made and all parties agree, complete the Request to Use a Non-Registered Interpreter/Transliterator.
 
If a person who is deaf, deafblind, or hard of hearing decides to revoke (cancel) their request to use a non-registered interpreter, the following form must be completed: Revocation of the Request to Use a Non-Registered Interpreter/Transliterator.
